Don't know if this is of any value... but the latest ver. of OpenOffice.org saves in a docbook format, maybe this would help.
Also... PhpDocumentor's 1.2.2 docbook/peardoc2 is phenomenal.
Finally... maybe docbook isn't the way for PEAR. Sure it'de be a pain to change anything, but if as many developers as it seems distaste it so much then maybe something else needs to be considered.
The other option is... Someone writes a really good docbook prog in PHP-GTK and make every bodies life real easy. :-)
