For the record, zend_file_handle has not been changed. Zeev
is obviously looking at some old version and not at the current
code.
Regarding changed semantics - yes, there is some potential
that old engine code has been copied to extensions. That is
however an extremely hypothetical case.
Extensive tests with the Zend Performance Suite, PHPA, APC 2
and the Turck cache have all been successful so far without
exception.
