If you attempt to install File_CSV, and it has a required dependency like so:
<dep type="pkg" rel="ge" ver="1.0.4">File</dep>
it will fail unless File's version is greater than 1.0.4.
$ pear install File_CSV
Error: File_CSV requires File >= 1.0.4
install failed
To ensure that people who wish to get the functionality of File_CSV by installing File will get that, it just needs an optional dependency on File_CSV. In this situation, on installation of File, people will see:
$ pear install File
`File_CSV' is recommended to utilize some features

Greg, might this work as expected?This is a good point - the uninstaller doesn't check for conflicts of filenames during uninstall currently, so if you install File_CSV first, and uninstall File, it would delete File/CVS.php. The required dependency in File_CSV is the solution, as it should not be installed with older versions of File, since the older versions would have File/CSV.php anyways.
