I also like Christian's idea, it makes it very comfortable to use.
So we need two new classes for the header and the footer, both having
functions like setCellContents(), setRowType() etc.
When the third constructor param is true, an instance of the header and
footer classes will be generated.
For the "normal" (t)body rows $_structure and $table->setCellContents() ...
could be used like before.
Or do you also want also want a third class (for <tbody>), and let
HTML_Table ignore content added directly with $table instead of $body?
