phpDocumentor has been able to produce fully compatible peardoc2 from inline source without modification since version 1.2.2. 1.2.3 fixed one bug with <optional> not being closed in function parameters.
The way you use it is you generate docs directly into en/package/, run make html and check out the result. Modify as desired.
Not hard.
PEAR_PackageFileManager has docs generated directly.
