It should be DB/NestedSet.php, not DB/DB_NestedSet.php.
DB_NestedSet is the name of the class, not the name of the file.
1. Create a directory in CVS called DB_NestedSet
2. Put a NestedSet.php file in it.
3. Define your install path to DB/NestedSet.php in your package.xml
Look at other packages like DB_Pager to see how it's done.
