There are some starting points available around.
I use the "Secure User Login System" from (and used by) phpbuilder.com:

and it works very well, but is limited on the features side (no deleting user
for example).
A guy wrote a little extension to add as much fields you need :

Its not PEARized at all (I just added a PEAR DB access), but its a set of
functions not liked to any HTML production system.
