Allowing the user notes to float up to the right of the main content would be ideal for me. Also when it comes to printing pages, having a choice to show user notes or not would be ideal also. http://www.w3.org/TR/REC-html40/present/styles.html#h-14.3.1 provides the mechanism to allow multiple style sheets. I'm not sure if you can attach the media="print" attribute to these and still have them working? WIth the increased use of small screen devices, I also think style sheets for media="handheld" should be looked at. And of course, the accessibility issues for screen readers.
