Okay, figured this out.
During install, I changed point 3: "3. PHP code directory" from the
default to "$prefix\PEAR". This causes the installer to NOT create a
pear.bat.
However, even when I leave everything set to the defaults, pear.bat does
not work.
When/If it's created, it contains:
set PHP_BIN=@php_bin@
set BIN_DIR=@bin_dir@
set PEAR_PATH=@include_path@
Ie. @php_bin@ isn't replaced by the real values.
But right now, I'm unable to get the go-pear installer to create this
batch file. When it installs PEAR 1.0.1, a pear.bat is created. After
the installation, the (wrong) warning message about the include_path not
containing the configured directory is shown.[1]
After this, the script seems to check if there's a pear.bat in the PATH
and finds the one it just created. Because it found a pear.bat it moves
it away to pear.bat_old.

That's, because on line 345 of the PEAR installer script, it does:
if (!in_array($php_dir, $include_path)) {
Well, $php_dir isn't in $include_path - if letter case would be
important. However, we're on Windows, and thus the letter cases have to
be ignored (ie. 'C:\PHP' == 'c:\php'). This could be done, by inserting
these lines right before line 345:
$_i = array();
foreach ($include_path as $_inc_path){
$_i[] = strtolower($_inc_path);
}
$include_path = $_i;
unset($_i);
if (!in_array(strtolower($php_dir), $include_path)) {
